Chronic Care Management (CCM) is a new model of care designed to improve the coordination of healthcare with an emphasis on your overall well-being. If you are a patient with two or more chronic conditions, you may benefit from this new program that Georgia Renal Specialists offers to all Medicare beneficiaries. The goal of the program is to make sure you get the best care possible from everyone that is involved with your care. We believe that to achieve this goal there must be a partnership between the patient and their medical provider. By remaining involved in the decisions regarding your healthcare and lifestyle, we can develop a stronger relationship with you.
